Source text - English CYSTIC FIBROSIS (CF)
Cystic fibrosis (CF) is the most common
recessive genetic disorder found in
Caucasians with an incidence of 1 in
2,500 live births.
The main clinical symptoms are
characterized by functional abnormalities
in the airway epithelium, the exocrine
pancreas, the gastrointestinal tract and
the secretory duct of the sweat gland,
leading to pancreatic and pulmonary
insufficiency.
In 1989, the gene responsible for CF
was identified by positional cloning.
The product of this defective gene was
termed the cystic fibrosis transmembrane
conductance regulator (CFTR) (1,2). The
CFTR gene is composed of 27 exons,
spanning 250 kb on the long arm
of chromosome 7, and encodes a
membrane associated protein of 1,480
amino acids. The CFTR functions as
a C1 - - ion selective channel, which
is activated by both protein kinase A-
mediated phosphorylation and binding of
The most common mutation causing CF
is a three-base pair deletion resulting in
the loss of a phenylalanine residue in
position 508 (ΔF508) (2,3). There is a
difference in the frequency of the ΔF508
depending on the population.
The frequency is about 50% in Southern
Europe, about 90% in the most northern
part of Western Europe, and about 70%
in North America (4).
There are now over 700 mutations
identified in the CFTR gene.
Diagnosis of CF is suggested by
manifestations of chronic sinopulmonary
disease and exocrine pancreatic
insufficiency. The diagnosis should
always be confirmed by a sweat test (5).
CF is not always readily identified in
infants and children, and considerable
delays in diagnosis after onset of
symptoms have been documented.
The identification of increased
immunoreactive trypsinogen (IRT) levels
in blood of infants with CF has made
neonatal screening for CF on a large-
scale possible (6,7,8,9,10,11).
Trypsinogen is one of the principle
members of the secretory products of the
human pancreas making its level in blood
a specific marker of pancreatic function.
Of the pancreatic enzymes, trypsin is
the only one produced solely by the
pancreas.
The acinar cells of the pancreas secret
two major isoenzymes of enzymatically
inactive trypsinogen (MW ~ 24,000),
which are activated to trypsin upon
cleavage of a hexapeptide from the N-
terminus.

Jericho Swain defeated Keiran Darkwill in a duel contesting the leadership of Noxus last night, winning the support of High Command. Despite rumors of involvement with the Institute of War’s scandal in Kalamanda, Swain has been named Grand General—the highest position in Noxus.
High Command remained divided in support between the two men since Swain returned earlier this month. Keiran Darkwill challenged Swain to a duel to settle the dispute, claiming that such a standstill was a “coward’s game.” Many members of High Command believed this to be a strong move and expected Keiran, a notoriously skilled duelist, to win.
Onlookers reported that Keiran initially had the upper hand and gradually forced Swain into a corner. “He had the Master Tactician against the wall,” stated one witness. “I heard him say ‘watch closely,’ and he vanished—then reappeared behind Keiran. Swain dragged him to the ground and locked the poor fool in his snare spell.” With Keiran immobilized Swain called General Darius, his most prominent supporter, to his side.
“Swain declared victory and left Darius to ‘evaluate the strength’ of Keiran Darkwill,” explained Chancellor Malek Hawkmoon. “That didn’t take Darius long. Without a word, he raised his axe—and removed Darkwill’s head.”
