Depression is an important and common nonmotor feature of Parkinson’s disease (PD) that is associated with significant disability and a negative impact on quality of life. The physician should remain vigilant for symptoms of depression as they may be mistaken for the progression of Parkinson’s disease itself. Transient dysphoria that occurs during ‘off’ periods in fluctuating PD patients must be distinguished from true depression. Antidepressant therapy should be instituted if depression is interfering with the patient’s daily function. The use of serotonin reuptake inhibitors and tricyclic antidepressants in the treatment of depression in PD is widespread in clinical practice. Dopamine agonists may be effective in the treatment of milder depression as well. Individual or family counseling may be helpful. In patients with severe depression who are refractory to antidepressant medications, a series of electroconvulsive treatments can be lifesaving. Nonconventional therapies such as transcranial magnetic stimulation are being investigated.

A depressão é uma característica não-motora importante e comum no mal de Parkinson (MP) que está associada com debilitamento significativo, e tem um impacto negativo na qualidade de vida. Os médicos devem estar atentos aos sintomas da depressão, já que estes podem ser confundidos com o próprio progresso do MP. A disforia que ocorre em períodos ‘off’ em pacientes parkinsonianos com sintonia flutuante deve ser distinguida da verdadeira depressão. Terapia com antidepressivos deve ser utilizada se a depressão estiver interferindo nas atividades diárias do paciente. O uso de inibidores da recaptação de serotonina e de antidepressivos tricíclicos no tratamento da depressão no MP é bem difundido na prática clínica. Agonistas da dopamina também podem ser eficazes no tratamento de depressão menos profunda. Apoio psicológico individual ou à família pode ser útil. Para pacientes com depressão profunda que são contra medicamentos antidepressivos, uma série de tratamentos eletroconvulsivos pode salvar vidas. Terapias não-convencionais, tais como estímulo magnético transcraniano, estão sendo investigadas.
