4.4 Speciale waarschuwingen en voorzorgen bij gebruik
Overgevoeligheidsreacties voor hydrochloorthiazide zijn waarschijnlijker bij patiënten met een
voorgeschiedenis van allergieën waaronder overgevoeligheid voor sulfonamide-afgeleiden.
Patiënten met een risico op een verminderde nierfunctie
Sommige patiënten bij wie de nierfunctie afhankelijk is van de aanhoudende inherente activiteit van
het renine-angiotensine-aldosteronsysteem (bijv. patiënten met ernstige hartinsufficiëntie [NYHA –
classificatie: klasse IV], bilaterale nierarteriestenose of nierarteriestenose van een solitaire nier)
lopen tijdens de behandeling met een angiotensine-converting enzyme (ACE)-remmer risico op het
ontwikkelen van oligurie en/of progressieve azotemie en in zeldzame gevallen acuut nierfalen. De
kans op het verkrijgen van deze verschijnselen is groter bij patiënten die gelijktijdig met een
diureticum worden behandeld. Bij deze patiënten die hier gevoelig voor zijn, is onvoldoende
therapeutische ervaring met angiotensine-II-receptorblokkers, zoals eprosartan, om te bepalen of
er een vergelijkbaar risico is op het compromitteren van de nierfunctie. De nierfunctie moet bij
deze patiënten nauwkeurig gecontroleerd worden, omdat er een verhoogd risico is op ernstige
hypotensie en nierinsufficiëntie.

4.4 Special warnings and precautions for use
Hypersensitivity reactions to hydrochlorothiazide are more likely in patients with a history of allergies including hypersensitivity to sulphonamide-derived substances.
Patients at risk of renal impairment
Some patients whose renal function is dependent on the continued inherent activity of the renin-angiotensin-aldosterone system (e.g., patients with severe cardiac insufficiency [NYHA-classification: class IV], bilateral renal artery stenosis, or renal artery stenosis of a solitary kidney), have risks of developing oliguria and/or progressive azotaemia and rarely acute renal failure during therapy with an angiotensin converting enzyme (ACE) inhibitor. These events are more likely to occur in patients treated concomitantly with a diuretic. Angiotensin II receptor blockers such as eprosartan have not had adequate therapeutic experience to determine if there is a similar risk of developing renal function compromise in these susceptible patients. Renal function should be monitored closely, because there is an increased risk for severe hypotension and renal insufficiency in these patients.

Source text - English Pregnancy and breast-feeding
If you are pregnant or breast-feeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
It is very important to stop smoking during pregnancy as it may result in poor growth of your baby. It can also lead to premature births or stillbirths. Ideally you should try to give up smoking without the use of medicines containing nicotine. If you cannot do this, Exmo Frukt should only be used after consulting the healthcare professional who is managing your pregnancy, your family doctor, or a doctor at a centre that specialises in helping people quit smoking.
Exmo Frukt should be avoided during breast-feeding as nicotine passes into breast milk and may affect your child. If your doctor has recommended you use Exmo Frukt during breast-feeding the chewing-gum should be taken just after breast-feeding and not during or before breast-feeding.
Driving and using machines
There are no known risks associated with driving or using machinery when using Exmo Frukt.
Exmo Frukt contains butylated hydroxytoluene. Butylated hydroxytoluene may cause local skin reactions (e.g. contact dermatitis) and local irritation to the mucous membranes in the mouth.
Exmo Frukt contains maltitol and sorbitol. If you have been told by your doctor that you have an intolerance to some sugars, contact your doctor before taking Exmo Frukt.

Source text - English 4.2 Posology and method of administration
Posology
The dosage depends on the intensity of pain and the patient’s individual susceptibility to the treatment. The following general dose recommendations apply:
Adults and adolescents over 12 years of age
Dose initiation
In general, the initial dose for opioid naïve patients is 5 mg oxycodone hydrochloride given at intervals of 6 hours. Patients already receiving opioids may start treatment with higher doses taking into account their experience with former opioid therapies.
Patients receiving oral morphine before oxycodone therapy should have their daily dose based on the following ratio: 10 mg of oral oxycodone is equivalent to 20 mg of oral morphine. It should be noted that this is a guide to the dose of oxycodone hydrochloride capsules required. Inter patient variability requires that each patient is carefully titrated to the appropriate dose.
Dose adjustment
Increasing severity of pain will require an increased dose of /.../. The dose should be carefully titrated, as frequently as once a day if necessary, to achieve pain relief. In doing so, the dosing interval may be reduced to 4 hours. The correct dose for any individual patient is that which controls the pain and is well tolerated throughout the dosing period.

De juiste dosering voor een individuele patiënt is die dosering die de pijn onderdrukt en goed wordt verdragen tijdens het gebruik.
