Se realiza estudio de Resonancia Magnética abierta de la muñeca y parte del antebrazo izquierdo con cortes en el plano sagital de 4 mm de grosor en las secuencias SE T1, FSE T2 y T1 con medio de contraste paramagnético y cortes de 3 mm de grosor en la secuencia STIR. Se realizan cortes en el plano coronal de 3 mm de grosor en las secuencias SE T1, gradiente T2 y STIR. Se realizan cortes de 4 mm de grosor en el plano axial en las secuencias SE T1, FSE T2 y T1 con medio de contraste paramagnético tanto en antebrazo como en muñeca. Se realizan cortes de 3 mm de grosor en el plano sagital en la secuencia SE T1 y en el plano coronal en la secuencia SE T1, además cortes de 4 mm de grosor en el plano axial en las secuencias SE T1 para el primer dedo de la mano izquierda.
EVALUACION:
Imágenes evaluadas en estación diagnóstica Radworks® con pantalla plana de alta resolución.
Adyacente al hueso escafoides, a nivel tendón extensor largo del pulgar se observa alteración en la intensidad de la señal que mide aproximadamente 1 cm de diámetro cráneo caudal x aproximadamente 7 mm de diámetro transverso el cual es hiperintenso en la secuencia T1, T2 y STIR, datos en relación con cambios de tipo crónico (fibrosis) en relación con la tenorafia antigua. No se logró identificar imagen que sugiera la presencia de cuerpo extraño a este nivel. No se observa colección líquida en relación con el tendón descrito. Se realiza análisis focalizado del tendón flexor largo del pulgar, del tendón extensor largo y corto del pulgar sin evidenciarse otras alteraciones.
Translation - English SEQUENCES: An open Magnetic Resonance Study of the wrist and part of the left forearm is performed, with slices in the sagital plane of 4 mm in thickness in the sequences SE T1, FSE T2 and T1, with paramagnetic contrast medium, and slices of 3 mm in thickness in the STIR sequence. Slices are performed in the coronal plane of 3 mm in thickness in the SE T1 sequences, T2 and STIR gradients. Slices of 4 mm in thickness are performed in the axial plane in the SE T1, FSE T2 and T1 sequences, with paramagnetic contrast medium whether in the forearm as well as in the wrist. Slices of 3 mm in thickness are performed in the sagital plane in the SE T1 sequences, and in the coronal plane in the SE T1 sequence, besides from slices of 4 mm in thickness in the axial plane in the SE T1 sequences for the first finger of the left hand.
EVALUATION:
Images evaluated in the Radworks® diagnostic station, with high-resolution flat screen.
Adjacent to the scaphoid bone, at the level of the extensor pollicis longus tendon of the thumb, there is alteration in the intensity of the signal observed, which measures approximately 1 cm in its cranial caudal diameter x approximately 7 mm in its transversal diameter, which is hyper intense in the T1, T2 and STIR sequences, data in relationship with chronic type changes (fibrosis) in relation to the ancient tenorraphy. An image which would suggest the presence of foreign body at this level was not identified. There is no fluid collection observed in relation to the described tendon. A focalized analysis of the flexor pollicis longus tendon of the thumb is carried out, of the extensor pollicis longus and brevis tendons of the thumb without evidencing other alterations.
