1.Przedmiotem niniejszej umowy najmu jest pokój o powierzchni użytkowej 18 metra kwadratowego w lokalu mieszkalnym położonym w Warszawie przy ul. Al. Ujazdowskie 34 m. 21 zwany dalej przedmiotem najmu.

Translation - English 1. SUBJECT OF LEASE AGREEMENT
1. The subject of this Lease Agreement shall be a room of usable area 18 square meters in an apartment located in Warszawa at Al. Ujazdowskie 34 flat 21, hereinafter referred to as the Subject of Lease.
2. The Lessor represents that he is the owner of the Subject of Lease, and in witness whereof he is awaiting: a certificate by the Building Society, a copy of which constitutes an Appendix hereto.
3. The Subject of Lease is equipped with the efficient wiring system, television system and heating system.
4. The Lessor leases for the benefit of the Leaseholder the above mentioned Subject of Lease (with furniture) jointly with bathroom fittings and access to the kitchen and gas-fittings.
5. The Subject of Lease shall be released to the Leaseholder with a record of release-and-receipt of the rooms, with a specification of the technical condition of the rooms, its fixed equipment and a detailed description on July 07, 2005.

Source text - English In addition to the policies, procedures and forms identified elsewhere in this booklet, the Company has established these general guidelines which apply to work-related injuries/illnesses and the workers’ compensation program:
• If you are injured from a work-related incident, you will be paid for the time missed from work for a medical appointment with a company-designated doctor on the day of the incident provided that you return to work that same day, or no later than the start of the next scheduled work day.
• If you are injured from a work-related incident and have medical restrictions that require modified work assignments, you will be paid your pre-injury average weekly wage and will not have a wage loss provided that you work a regularly assigned full-time work schedule.
• If you need transportation to your first medical appointment, the Company will provide it. However, it is your responsibility to obtain transportation to follow-up medical appointments. Please schedule follow-up appointments “off work” hours when at all possible.
