Ovde je rec o vrsti upitnika koji se koristi za procenu kvaliteta zivota pacijenata sa hronicnim oboljenjima, pa je rec o tzv. kratkoj formi upitnika za procenu QoL (kvaliteta zivota). Radim ovo gotovo svakodnevno, tako da sam sigurna da je rec o SF (short form) of inventory. Nije los ni predlog "instrument", ali obicno koristimo u klinickim istrazivanjima "KRATKA FORMA"...
