Se busca propiciar en los niveles provinciales, acciones de planeación estratégica que asuman la prioridad del cumplimiento de los derechos humanos de las mujeres, que promuevan una mejora sustantiva en la oferta de servicios de salud, la universalidad en el acceso de las mujeres a los mismos ****y un decidido fortalecimiento de la demanda, ****empoderando, para ello, a las mujeres de las regiones más pobres.

Here's what I have so far, but it doesn't sound quite right to me.

Strategic planning actions are needed at provincial levels which champion the fulfillment of women’s human rights as a priority, which promote a substantial increase in available health services, universal access to them for women, and a decided strengthening of demand, and by doing so empowering women in the poorest regions.

I'd be grateful for any help....TIA

I think it sounds off to you because the whole thing has too many nouns.
Try:
"... that seek to markedly expand available health services, promote universal access for women, and significantly boost demand ...

To "boost demand" is an incredibly common collocation.
Example sentence:

... the federal government could significantly boost demand for alternative fuels ...

you have it right but I'd put the rest of the sentence this way:

empowering women in the poorest regions to achieve that OR
achieved by empowering women ...

Also, I'd use "improvement in health services" rather than "increase"

In Orellana's Glosario internacional del traductor, both "strengthening" and "building" are given. This would be another option.
