Here's what I have so far, but it doesn't sound quite right to me.
Strategic planning actions are needed at provincial levels which champion the fulfillment of women’s human rights as a priority, which promote a substantial increase in available health services, universal access to them for women, and a decided strengthening of demand, and by doing so empowering women in the poorest regions.
I'd be grateful for any help....TIA

I think it sounds off to you because the whole thing has too many nouns.
Try:
"... that seek to markedly expand available health services, promote universal access for women, and significantly boost demand ...
To "boost demand" is an incredibly common collocation.
Try:
