"Protruding" would be the standard word if the gauze was left unintentionally; 'inserted' would be more appropriate if the gauze was left in the wound intentionally, as to absorb blood.

I just looked up "insinuate" (which is all I can get for "insinuar") in an English dictionary, and it can mean to introduce or insert. I would say, looking at the context, that "inserted" is reasonably likely. HTH.
