I am translating a hospital form with a doctor's notes regarding EKG results for a patient.
Typed in the form is the following:
All’ecg tendenza al sopra st in inferiore con speculare lieve sotto st in laterale
I am not sure how to render the words "con speculare" in English. "...with suspected..."?
Any help would be greatly appreciated!

Not sure about this, but I've come across the term "specularità" in the past, and it might be this.
There are many online instances of "mirroring" in an ECG context, for example:
"The ECG changes seen in the standard leads are reciprocal, mirroring the ST segment elevation that was only visible when posterior leads (V7–V9) were ..."
Hope this helps, and hope somebody with real medical knowledge comes along to confirm or correct this.
