Dutch to English Medical Medical (general)
ECG bij presentatie: sinusritme HC, recent milde DM ontdekt.

ECG at presentation: sinus rhythm HC , recent mild DM discovered.

Anyone know what this HC might be? Looks like it could be a treatment (elsewhere: Rx: HC...)

The whole report is telegraphic.
So the sentence given: "ECG bij presentatie: sinusritme HC, recent milde DM ontdekt". With physical exam results before and lab test results after.
+ "Rx: HC, recent milde DM ontdekt, dieet behandling", in the same configuration...

Give as much context as you have, including the way things are presented, like the order of phrases.

Hypercholesterolemia

It could be this depending on the context, possibly if there was supposed to be a comma that was accidentally omitted between rhythm and HC. Dieetbehandeling would work for hypercholesterolemia as well, so it is a possibility.

neutral Maja Keizers (X) : I see your point, but please explain how hypercholesterolemia would show up on radiography ("Rx: HC"). As far as I am aware, only lab work could demonstrate hypercholesterolemia - the disease, not the resulting plaques.

neutral Barend van Zadelhoff : Not sure this fits the context. Also, acronym of 'hypertrofische cardiomyopathie' would be 'HCM'//It's just a guess, Maja, and if there was HCM, you would not record it this way. Also, 'sinusritme' is just to a snapshot here.//English acr is 'HCM' as well

HC may lead to vfib so it is reassuring to know there is sinus rhythm. It will show up on ECG and on echocardiography and possibly on x ray. As to the actual acronym, it would not be a first to see an English acronym used in a Dutch report. Whatever.
