medical journal
This is the very first line in an infant's medical journal from Karolinska:
date, time, physician's name, specialty, and then "Stängd SU -HPKlinNutrition (signerad)"
"Klin(isk )Nutrition" seems self-evident, as does "signerad", but what about the rest?

Discussion
e.g. date X, time Y, physician Z, and then "Diet Stängd S - Dietistklin, Barn (signerad)"
or date A, time B, physician C, and then "Diet H - Dietistklin, Barn (signerad)"
This part of the journal is signed by a diet(itian). Clinic is "Neo, Solna".
