Comment: "Thank you all for your carefully crafted answers. It's true that Taxpayer is usually "contribuinte". Maybe, here, they wanted to differentiate between indiviuals and corporations. I thought, originally, that it couldn't just be "Taxpayer Identification", since it seemed to be a heading in the document. On reflexion, it is probably just a note as if to say "Insert Taxpayer ID".
