In 2005, France implemented a gatekeeping reform designed to improve care coordination and to reduce utilization of specialists’ services. Under this policy, patients designate a médecin traitant, typically a general practitioner, who will be their first point of contact during an episode of care and who will provide referrals to specialists. If a patient has not registered with a preferred doctor (usually a GP), has registered with a preferred doctor but nevertheless visits another GP or visits a specialist without a GP referral, the SHI rate of coverage drops from 70% to 30%. |

Registration with a primary care provider (family physician (US)/GP (UK)) maintained Explanation: Anyone satisfying certain conditions, but more or less everyone, has a right to medical care in France, including primary care, but a patient can choose to name or appoint a specific practice / physician as his/her "medecin traitant" , or primary care provider of choice, and hence be registered with that practice/doctor. Note that I said "a" meaning that under this system you can keep having a "medecin traitant" but change practice/doctor. |
