The term "immunization" is more commonly used than vaccination (at least in the US).

I think that this is what we would say. Hope it helps.

I wonder also whether or not the original is not using "schéma" to mean "cas de figure où...", by way of example. It is using it by way of comparison, which is fairly close in meaning. If so, then you might like to express it a little more loosley, "in a situation where children are vaccinated at 0, 1 and 6 months".
