I think you should use media order if you're not sure whether this refers only to print media.

Insertion Orders

Insertion orders are due for processing as soon as possible after the insert is booked in order to hold the space.

Insertion orders should contain the following information:

Name of advertiser.
Insertion date.
Contact name at agency/printer.
USA TODAY sales representative.
Number of pages per supplement.
Length and width of supplement.

Because "insertion" would exclude broadcasting and internet. Below is a link to a very neat glossary of media terminology. TV only could also be called "programming schedule".
