This is difficult, because perhaps the fact that the patient was being discharged meant that they were in fact tolerating the requisite post-procedure diet for discharge.

However, if the "sin" is absolutely to be included, then perhaps the patient is being discharged despite not having been able to tolerate this diet.

It seems there may be a standard diet that they use as a determiner when considering discharge, or perhaps it's one personally recommended by the doctor.

May need clarification on that.

(1) The patient might be getting sufficient nutrition but still not well tolerating the diet. He may suffer nausea, for example. (2) There will no doubt be followup, at which time further decisions may be made. (3) After all, the patient did not expire. Possibly the patient does not have sufficient financing for further hospitalization unless absolutely necessary.

Please, remember how Medical reports are written. They go from verbal to manual writing or recording, and then to typing.
Mistakes can happen. It would not be the first time that something is written because it just crossed the mind of the typist then, making no sense at all.
Let's work it out.
PO= Procedimiento Operatorio (Operating Procedure)
sin= typing mistake
It would not make sense to send the patient home without tolerating the diet. In fact, it is the other way around. Patients are discharged from the hospital when conditions allow the patient to be reasonably cared for at home.
Double check.

I was doing a bit of researh on Mexican search engines and the way it is usually phrased is like this:
egresándose a su domicilio al sexto día (or whatever number of days, and they are always written out) posteriores a su ingreso tolerando la dieta
so I am going to make a wild guess here and say that PO is posteriores and "sin" is (S)u (IN)greso.
Do you know how many days was this guy in the hospital?

I agree with lorenab23. I think, based on the context, that the patient in fact WAS tolerating the diet, so maybe they were trying to say "tolerando la dieta" (tolerando la dieta).

may not mean without, it may be part of PO. it seems to me that they are saying "tolerando la dieta". and the verb "tolerar" is being conjugated just like "efectuando", "evolucionando", "egresando(se)"...
Previously he was not tolerating food, now he is
