En el estado asmático, la enorme resistencia al flujo a través de la vía aérea produce, si se utilizan flujos inspiratorios elevados, una gran elevación de la PIP con aumento del gradiente entre la PIP y la Pmeseta. Sin embargo, la PIP elevada no predice la aparición de barotrauma durante la ventilación mecánica del estado asmático . De hecho, la estrategia que trata de minimizar la HID resulta en ocasiones PIP muy elevadas (hasta 60 cmH2O).
