The writer of a medical report is listing the people he consulted regarding care of a certain terminal patient with melanoma, and one of these is Prim. XXX.
Wild guess: Chief Physician (in the Hematology Department).
I do not think it means "the patient's primary care physician". I think it is a title.

"Primarius/Primar" = Austrian speak, same as "Chefarzt" in German...

: wouldn't go with senior consultant. A primarius actually RUNS a department-- albeit at a "non-university" hospital.

Primarius= chief of staff
Ordinarius= head of department
Another note on this issue, perhaps: in some other EU countries, however, Primarius is NOT the head of department, but has achieved recognition for his/her clinical work/achievement and granted this title.
So, translate as "chief of staff" ONLY when the language pair is DE_EN!!
