neutral  philgoddard: Yes, it could mean "provider", but we can't be sure.

  -> I know, but because it appears at the top of the form I'm inclined to think it refers to the body that provided the medical care, in this case the Catalan Medical Services; not the services provided by a private clinic.

Explanation:
There are some hits for this target term with reference to Valencia and Catalunya is right next door. I wouldn't be surprised if this abbreviation (my suggestion in castellano) could stand for the same notion in Catalan.

Explanation:
Por este motivo, todo apunta a que se refiere a "Aseguradora" (Insurance Company). Porque no es un acrónimo relativo a dos términos (por ejemplo "Agencia Sanitaria" o "Asistencia Sanitaria"), sino que es un sólo término, que habitualmente se simplifica así.

Normalmente en la parte superior de los informes de pacienes se suele incluir el nombre de la Aseguradora que presta el servicio a dicho paciente (Mapfre, u otras similares, etc. -por no hacer publicidad no sigo).
En el caso de los adscritos al régimen general español, en lugar de una Aseguradora Privada la asistencia la brinda el Servicio Nacional de Salud español, que en cada Comunidad Autónoma está gestionado (y el servicio es prestado) por un organismo del Sistema de Salud Comunitario correspondiente; en este caso, el Servei Català de la Salut.
Respecto a "Agencia Sanitaria", en castellano no se utiliza y tampoco en catalán.
