or under nurse's care - depending who has been taking care of the patient.
If you want to use "follow", you can say, "Under follow up care". But follow up usually does not last 3 years. It's a one-time or a very short type of care.
Eg. The patient has been under the care of physician/nurse/clinic for the (describe injury) rehabilitation for the last three years.

P.S. "Follow up" does not mean the same in English as "наблюдение" in Russian. Follow up (FU) means that a doctor does a surgery and makes an appointment with the patient to see if there is no infection, etc. Or a surgeon may sand the patient to another specialist for a follow up.
Наблюдение is more of a long-term monitoring and care done by a physician (not surgeon, typically), nursing care, physical therapy, etc.
