Estoy traduciendo un informe neurológico de una paciente que sufre pérdida de conciencia:
"... se reintenta omitir fenobarbital (el paciente refiere reinicio del tratamiento automedicado por miedo a crisis) durante este período, presenta problemas laborales por la inatención y trastornos de memoria, ameritando intervención del área laboral, por el servicio de trabajo social y psiquiatría, quien indica tratamiento ansiolítico. Se plantea la posibilidad de **trastorno cognitivo de tipo farmacológico" y se insiste con la omisión de fenobarbital progresivamente. Acude a controles durante los siguientes años persiste anictal sin cefaleas refiere persiste inatención y conductas afectivas de tipo maneiformes [sic] que no alteran su vida cotidiana, actualmente persiste recibiendo fenobarbital 50 mg y memantina 10 mg/día sin otro particular y mantiene en control médico."

Most commonly known as drug-induced cognitive impairment but just to avoid confusion between drugs (as in street drugs) and drugs (as in pharmaceuticals) I would use this.
Because medication-induced cognitive impairment is often reversible, it is important to conduct comprehensive medication reconciliation in order to provide for early identification and withdrawal of the offending agent(s).
