I think that one option could be "as disclosed by" another option could be "pursuant to information contained in..."

I also like "as disclosed by"... but the usual translation of "constar" is record/register, which are the first options that sprang to mind

By the way, you should delete the 'he' in 'he has not been' as it should read 'That JUAN PEREZ...has not been penalized' and not 'JUAN PEREZ...he has not been penalized'.
