I take it Wirkorgan doesn't mean that the lungs cause (i.e. bewirken) the inflammation (SIRS)?

It definitely does not mean that the lungs cause the inflammation but rather that they are affected by it (which may lead to lung failure).
I suggest you say: "[...], the lungs function as the primary target and effector organ"... rather than saying 'are primarily affected as'...

For starters, I'm thinking alongthe lines of: Depending on the severity of the SIRS, it is the lungs that are primarily affected as the central Ziel- und Wirkorgan...

"affected organ" would refer to the effect on the organ as well as its resulting function and effects thereof
