Este ca un pliant, unde pe prima fata apar datele tale (nume, nr istorie, adresa, tel), iar pe cealalta parte se trec datele si orele vizitelor posterioare. Ca un fel de pasaport cu vize, sa zicem. Nu este acelasi lucrui ca si carnetul de sanatate. Eu i-as zice card ambulatoriu.
