Source text - English 1. The Trial
1.1. It is agreed that the Institution and the Investigator shall be collectively referred to hereunder as the “Site”.
1.2. The Site undertakes that the Trial shall be conducted in compliance with (i) the Protocol; (ii) the instructions and terms specified in the EC's (as defined below) approval; (ii) the terms of this Agreement and Sponsor's written instructions; and (iii) all Applicable Laws.
1.3. For the purposes of this Agreement, the term “Applicable Laws” shall include all national, federal, community and local applicable laws, rules regulations and guidelines applicable to clinical trials and the ethical principles and the code of conduct inspiring the medical activity and anti-corruption legislation, and in particular (i) the regulations and requirements of the Regulation (EU) 2017/745 and/or the Directive 93/42/CEE, if applicable, the regulations and requirements of the US Food and Drug Administration ("FDA"), and/or any other regulatory authority having jurisdiction over the conduct of the Trial ("Regulatory Authorities"); (ii) the World Medical Association Declaration of Helsinki regarding “Ethical Principles for Medical Research Involving Human Subjects” as amended and modified from time to time and all applicable standards and guidelines for Good Clinical Practice (“GCP”), including, without limitation, generally accepted standards of GCP as set forth in Title 21 of the U.S. Code of Federal Regulations (“C.F.R.”), ICH GCP E6 and ISO 14155 “Clinical investigation of medical devices for human subjects”; (v) all conditions of approval imposed by the reviewing Ethical Committee ("EC") and/or other a authorities; and (v) all other applicable local, national and/or international laws, rules and regulations including, but not limited to, those applicable to the conduct of the Trial and/or the activities to be performed under this Agreement, and/ or governing the use or review of data obtained as a result of the Trial.
1.4. Should any contradiction arise between the Protocol and this Agreement, the terms of this Agreement shall prevail. Any amendment to the Protocol shall be submitted to the prior review by and written approval of Sponsor prior to its submission for approval to the EC.

Військові підрозділи впродовж тривалого часу беруть участь у безперервних бойових діях в особливих та екстремальних умовах. Ці дії змушують військовослужбовців виконувати обов’язки при несприятливих умовах, формують відповідну поведінку та викликають помилки в бойовий роботі, що призводить до виникнення бойового стресу.
Змінені психічні стани, що виникають у військовослужбовців в період ведення бойових дій, включають бойову стресову реакцію, бойову втому та посттравматичний стресовий розлад (ПТСР). Досвід показує, що психологічно неадаптивні реакції на бойові обставини складають від 10 до 50% всіх санітарних втрат в сучасних локальних війнах.

Ознаки та симптоми зміни психічного стану
Бойова і оперативна стресова реакція (combat and operational stress reaction) – це первинна нормальна реакція на ненормальний стрес, викликаний участю у бойових діях. Під час війни люди перебувають під постійним фізіологічним (наприклад, погане харчування, перепад температур, відсутність належної особистої гігієни тощо) та психологічним стресом (наприклад, постійна небезпека або ж поранення чи смерть товаришів). Тому у більшості з них виникатимуть ті чи інші психічні реакції. Ці реакції часто проявляються як підвищена полохливість (сильна реакція, наприклад, на гучний звук), надмірна пильність (боєць завжди на сторожі і готовий діяти), погані сни/жахи, дратівливість, проблеми зі сном і т.п. Хоча все це і звучить досить негативно, проте деякі з цих реакцій вважаються адаптивними, зокрема, підвищена полохливість і надмірна пильність, та й інші реакції бойового стресу, такі як зростання фізичної сили, збільшення витривалості, відчуття власної компетентності тощо (тобто не всі стресові реакції є негативними).
Термін «бойовий стрес» використовується для опису будь-якої пов’язаної з боєм стресової реакції, що потребує допомоги. Більшість військовослужбовців, у яких проявляються ознаки і симптоми емоційних чи психологічних розладів, не мають психічних проблем, вони, швидше, намагаються подолати аномальний стрес військових операцій. Це розмежування з психіатричними розладами дозволяє визначати бойовий стрес як нормальний стан, який розвивається у нормальних людей під впливом ненормальних обставин. Важливу роль відіграє здатність самих військових та їх найближчого оточення (насамперед командирів та співслужбовці) адекватно оцінювати стан людини, виявляти ті симптоми, які свідчать про розвиток стресу.
Бойовий стрес спочатку проявляється як індивідуальна нездатність людини виконувати свої прямі службові обов'язки. Серед найбільш характерних ознак необхідно відзначити покидання поля бою, нез'ясовану відсутність та інші. У тих військовослужбовців, які відчувають бойовий стрес, існує велика ймовірність повторення симптомів, тому чим невідкладніше буде надана допомога, тим більше шансів швидкого відновлення. Психологічні фактори (особистісні особливості людини) можуть затримати або ускладнити реабілітацію після таких станів. Будь-яка поведінка військового, яка тривалий час відрізняється від звичної для нього раніше, може бути ранньою ознакою і симптомом бойового стресу. Для своєчасної діагностики не обов’язково тримати в пам’яті всі можливі ознаки і симптоми бойового стресу, набагато кориснішим буде запам’ятати одне просте правило: знайте своїх хлопців і будьте готовими помітити будь-які раптові, тривалі чи прогресуючі зміни в їх поведінці, що несуть потенційну загрозу безпеці та функціонуванню вашого підрозділу. Процес відокремлення військових з бойовим стресом від відносно невеликої кількості тих, які мають реальні психічні розлади називається нейропсихіатричним сортуванням. Основний показник того, що спостережувана симптоматика відображає нейропсихіатричний рівень травматизації - збереження військовослужбовцем в достатній мірі критичної оцінки того, що відбувається і здатності до цілеспрямованої діяльності. Здійснюючи оцінку стану бійця в тактичних умовах завжди потрібно думати про БЕЗПЕКУ. Якщо виникають сумніви стосовно безпеки самого військового чи його оточення – потрібно переконатись, що у нього вилучена зброя і він знаходиться під постійним наглядом. Не використовуйте військовослужбовців з ознаками бойового стресу для евакуації поранених – це може підвищити рівень травматизації.

Military units are involved the continuous warfare actions in special and extreme conditions for a long time. These actions make servicemen perform their duties under unfavourable conditions, shape the respective behaviour and trigger combat performance issues, which lead to combat stress.
The mental state alterations that occur with servicemen during warfare include combat stress reaction, combat fatigue and post-traumatic stress disorder (PTSD). Experience has shown that psychologically non-adaptive reactions to combat conditions constitute 10–50 % of all sanitary casualties in modern local wars.
Teaching point No. 1:
Signs and symptoms of mental status changes
Combat and operational stress reaction is a primary normal reaction to abnormal stress caused by participation in warfare. During warfare, people are subjected to constant physiological (for example, poor nutrition, change in temperature, lack of proper personal hygiene, etc.) and psychological stress (for example, constant danger or a comrade’s injury/death). Therefore, the majority of them will have mental reactions of any kind. These reactions often manifest themselves as increased fear susceptibility (for example, a strong reaction to a loud sound), unceasing vigilance (the serviceman is always alert and ready to take action), bad dreams/nightmares, irritability, poor sleep quality, etc. Although all this sounds rather negative, some of these reactions are considered adaptive, in particular, increased fear susceptibility, unceasing vigilance and other combat stress reactions such as increased physical strength, increased endurance, feeling of self-competence, etc. (i. e., not all the stress reactions are negative).
The term “combat stress” is used to describe any case of combat-related stress reaction that shall be regarded by a professional. The majority of servicemen having signs and symptoms of emotional or mental disorders have no mental health problems, but rather try to overcome the abnormal level of warfare stress. This differentiation from mental disorders allows us to define combat stress as a normal condition that develops with mentally healthy people under the influence of abnormal conditions. An important role is played by the ability of servicemen and their immediate surroundings (especially their commanders and comrades) to adequately assess one’s condition and to identify the symptoms of stress development.
At first, combat stress is manifested through the inability to perform one’s direct duties. The most typical manifestations include leaving the battlefield, unexplained absence, etc. The servicemen having combat stress are more likely to experience recurrent symptoms; therefore, the more urgent help is provided, the better the chances of a speedy recovery are. The rehabilitation process after those conditions may be delayed or complicated by some mental factors (individual features of character). Any behaviour of the serviceman, which differs from his usual one for a long time, might be an early sign and symptom of combat stress. For timely diagnostics, it is not necessary to keep in mind all the possible signs and symptoms of combat stress. It is more useful to remember one simple rule: know your comrades and stay alert for any rapid, continuous or progressive changes in their behaviour, which pose a potential threat to the safety and functioning of your unit. The process of separating servicemen with combat stress from a relatively small number of those with real mental disorders is called neuropsychiatric sorting. The main indicator that the observed symptoms are not a sign of neuropsychiatric pathology is the serviceman’s preservation of a sufficiently critical assessment of what is happening and the ability to perform a purposeful activity. When assessing a serviceman’s condition in a tactical setting one shall always think about SAFETY. If there are doubts concerning the safety of the serviceman himself or his immediate surrounding, one shall make sure that the serviceman has no weapon and is kept under constant supervision. The serviceman with signs of combat stress is prohibited to evacuate casualties as it may increase the severity of his condition.
